It consists of a street outreach team, a one-stop center for homeless services, a year-round 40-bed emergency shelter program for homeless families and individuals and a transitional housing program for homeless families. Case management is a primary activity of this project and the key to its success. Moving beyond the provision of clients' basic temporal needs (food, clothing and shelter), case management assists clients to identify and remove barriers in order to achieve self-sufficiency and independence. They meet with specialized case managers in employment, housing, mental health, substance abuse recovery and veterans services when appropriate.
